my child kaitlyn cain is a danger to herself and brothers and the events of this past saturday 4-6-96 are proof. she is also a danger to myself and has made many death threats about me in front of to a trained professional from her school. i feel she also is an active danger to the household. i would need to have her transported to the scheduled doctors appointment on friday 4-12-96 from the respite home.
from the respite home at 2601 e oak. to her doctors appointment at 1 pm friday 4-12-96.
to 1101 e. missouri for her doctors appointment where i will meet her and the staff at 1 pm. I expect dr. silverman and com care to immediately give her out of home placement either in a hospital or group home as residential treatment center and will file complaints, grievences, or changes or whatever it will take for the safety of my family and her continued
i have been forced to this disgrace because of the inadequate care she received from com care in the past which lead to this immediate crisis.
